In our effort to balance teams for the upcoming season, we ask that you list the players on your team by consecutive jersey number, then rate each player in the categories listed below on a scale of 1 to 10 (1 being lowest and 10 being highest). Multiply the rating by the indicated factor to achieve rating for each category. Add ratings for each category to obtain total player rating. This information should be kept confidential, and will be used by AYSO for balancing teams next season. Please complete and submit this form before the last week of the season.
